The Nursing Home Transition Program's overall goal is to help individuals transition into the community from nursing homes. It is designed to assist consumers by working within the existing systems and connecting them with specific home and community based services an individual will need to be able to live independently in the community.

LR Marketing Group assisted four agencies, Abilities in Motion and the Berks, Lancaster, and Lebanon County, Pennsylvania Area Agencies on Aging, in promoting the program to consumers, family members, nursing home facilities, and service providers in the community.
